Providence is calling a Case Management Associate, LVN (Per diem, Day shift) at Providence Saint Joseph Medical Center in Burbank, CA.

We are seeking a Case Management Associate, LVN who will be responsible for assisting the case managers to identify the discharge needs of the patient and family and help facilitate a smooth and safe transition from the hospital. Responsible for assisting the case managers with all the discharge planning needs of patients that are assigned and utilizes the Case Managers for situations that are out of his/her scope of practice.
In this position you will:
Maintain established departmental policies and procedures, objectives, improving organization performance program, and safety standards
Interact with medical support staff and physicians assigned to individual patients to facilitate the discharge process (i.e., identify what needs the patient or family may have)
Oversee, with the instructions of a Case Manager, the appropriateness of stay in the hospital, both acute and skilled
Meet with multidisciplinary groups to discuss discharge plans on a daily basis
Participate in administrative staff meetings on a monthly basis and attends others as appropriate to enhance professional growth
Identify patient/discharge needs to assure that both receive support with resources and placement assistance
Assure appropriate documentation is completed on patient records as identified in department policies
Facilitate the transition home by ordering equipment, arranging and referring home health visits, contacting the HHS prior to discharge, and initiating a follow up telephone call after discharge
Current CA License Vocational Nurse
Experience in a health care related position
One (1) year experience in acute hospital case management & discharge
