Providence Health & Services Care Manager RN - Case Management in Burbank, California
Providence is calling a Full Time, Variable Shift Care Manager RN to Providence Saint Joseph Medical Center in Burbank, CA.
In this position you will:
Manage patient care, to achieve cost-effective utilization of resources through the proactive coordination of services in order to move patients efficiently through the continuum of care
Assume responsibility for an interdisciplinary process which assesses, plans, implements, monitors the effectiveness of interventions to meet patients’ treatment and transition of care needs
Required qualifications for this position include:
Nursing degree/diploma required upon hire.
All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree in Nursing or higher within 3 years of hire.
Any previously agreed upon effective date for obtaining a Bachelor’s degree in nursing between individual and facility will remain in effect.
Any collective bargaining contract with specific time frames for obtaining a Bachelor’s degree in nursing will supersede the above point and will remain in effect.
Exceptions will be considered on a case-by-case basis at each facility by the Director of Care Management in consultation with the Human Resources leader.
A RN with 20 years of RN experience will not be required to obtain a BSN.
RNs employed prior to 4/1/17 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.
Current California Registered Nurse license.
Current Basic Life Support Card (BLS) for Healthcare Providers sponsored by the American Heart Association.
Minimum of one year experience within the past three years in an acute hospital setting as Registered Nurse.
Minimum of one year experience as a Discharge Planner or Case Manager, successful completion of a minimum of 24 CEUs in Care Management within the past 12 months, or successful completion of the Transition into Practice (TIP) program for Care Management.
Strong clinical knowledge base.
Advanced knowledge of McKesson's InterQual SI/IS Criteria.
Preferred qualifications for this position include:
Bachelor's Degree in Nursing (BSN) strongly preferred
Minimum of 3 years experience in case management, discharge planning, and/or utilization review
